The Registered Nurse is responsible for establishing and implementing care plan, educating patients/caregivers and evaluating progress toward goals. Demonstrates competency in patient care standards required for safe delivery of services as evidenced by completion of orientation and nursing skills checklist.Will be competent in • the basic assessment of patients, • basic infusion skills, • basic wound care, and • completing clinical documentation. Manages Time effectively to deliver accurate and appropriate patient care. Arranges daily activities with regards to patient safety and team communication. Develops and implements a Plan of Care. Effectively supervises and coordinates paraprofessionals in the home (home health aide, homemaker)Effectively communicates with agency staff, medical team, patients, and family.Under the direction of the patient’s physician, safely performs and documents nursing care procedures in a manner consistent with scientific principles and agency policy.Prepares and submits documentation outlining services rendered within the required time frame. Communicates with patient’s physician and supervisor regarding problems, change in status, need for other team involvement, and goal attainment.Participates in multidisciplinary team conferencesMaintains Confidentiality of patient and proprietary informationObserve legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information.Qualifications:Minimum Requirements:• Graduate of an accredited school of nursing with valid license in state(s) of practice.• Valid and current driver’s license and evidence of automobile insurance.• Current CPR certification.• Minimum of one year of experience as a registered nurse.• Flexibility and the ability to work effectively within a multidisciplinary team• Ability to communicate effectively (verbal and oral) with team members, patients, caregivers, physicians and referral sources.Preferred Qualifications:• Previous Home Health Care Experience• Previous Infusion Experience• BSN.
