Commonwealth Care Alliance, Inc. (CCA) is a not-for-profit care delivery system committed to providing integrated health care and related social support services. Created in 2003, CCA is a “consumer-governed” organization offering a full spectrum of medical and social services for people with complex needs covered under Medicaid and for those “dually eligible” covered by both Medicaid and Medicare, including: older adults (65+) individuals with serious physical, cognitive, or chronic mental illness children with special needs

NURSE CASE MANAGER (CLINICAL COORDINATOR) - RN

 



Position Summary

 

Acts as a resource and liaison to Primary Care sites to ensure the goals of the CCA’s  clinical programs are met.

 

 

Permanent Essential Duties and Responsibilities

 Provide clinical direction to the contractual relationship between CCA and primary care sites.  (These will be designated clinical practices).Interface with primary care clinical staff on an ongoing basis to review care coordination, clinical decision making and pertinent clinical / administrative issues.Liaison with primary care staff to conduct case management activities.Review of patient / member charts and review of associated care plans.In this position, you will also be working with the provider network (e.g. hospitals, nursing homes, community agencies and hospitals), relative to quality or other administrative matters.In this position, you will be working in both our SCO and CCC/SNP Programs.Position requires the ability to travel to primary care provider sites within Massachusetts.

Requirements / Experience:

 

· BS in Nursing

· Licensure as a Registered Nurse

· 5-7 years minimum experience

· Experience in care of the elderly and other populations with special needs

· Experience in care management, utilization, levels of care and care delivery systems
