Nurse Assessor

Senior Health Partners (SHP) is seeking a dynamic, self motivated individual to join our growing organization as a Nurse Assessor (RN).  The Nurse Care Manager assesses the health status of our frail, elderly members, documents health status using the SAAM and reports back to our care management team regarding change in status or needs.

 

Requirements:

·  Requires license and current registration to practice as a Registered Nurse in the State of New York. BSN preferred.

·  Familiarity with the SAAM preferred.

·  Spanish, Russian, Cantonese or Mandarin speaking a plus.

·  Computer skills required.

·  Valid driver’s license and car required. (Mileage reimbursement provided)

·  Ability to work with an interdisciplinary care management team to provide services to frail elderly. 

·  Must be flexible and able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving.
