Mercy LIFE is seeking a full-time, Nurse Practitioner. The Nurse Practitoner provides primary medical management in collaboration with the LIFE Physician(s). Provides daily oversight of LIFE Clinic. Rounds in the LIFE contracted nursing home and hospitals. Conducts initial assessments and periodic reassessments, plan of care, coordination of 24 hour care delivery, regularly informing the IDT of the medical, functional, and psychosocial condition of each participant, remaining alert to pertinent input from other team members, participants caregivers, as well as documenting changes in a participant?s medical record consistent with documentation policies established by the medical director. Master?s degree in nursing, Gerontological preferred. Licensed RN in the State of Pennsylvania with current certification as a Nurse Practitioner. One year experience in working with geriatric population. Ability to work as a member of the multi-disciplinary team in a community-based setting and institutional health delivery system. Supervisory, leadership, and management skills. Understanding and willingness to learn and work with cultural factors that affect the health care of aging persons.
