NURSE PRACTIONER PARTNERS HOSPICE
Description:Under the supervision of the Hospice Medical Director the Hospice & Palliative Care Nurse Practitioner provides quality clinical consulting services to patients across diverse settings. Current, evidence based care is provided to those with chronic progressive or life limiting illness. All those affected by the diagnosis of a terminal illness including the person diagnosed, caregivers, close friends, and family, and community is considered appropriate recipients of care. Attention is on competent clinical management, informed decision making, safe environment and education regarding the terminal experience. The nurse practitioner also attends informational and educational events with other providers and educators as part of outreach responsibilities and as a means to provide current, evidence-based care.Qualifications:*Masters degree or higher in Nursing from an accredited school of nursing*Current certification as an Advanced Practice Nurse in Hospice and Palliative Care*Current registration in Massachusetts as a Nurse Practitioner*Current DEA and MSCSR or eligibility*Minimum of 1 year experience in oncology, community health or hospice and palliative care*Ability to travel within the geographic coverage area of Partners Hospice*Ability to synthesize complex data, implement advanced plans of care and provide leadership in hospice and palliative care*Ability to provide care in a collaborative manner across the life span and in diverse settings to patients and families experiencing life limiting progressive illness*Working knowledge of computer devices*Strong interpersonal skills*Ability to communicate effectively in writing and verbally with people of varied backgrounds*Ability to work as a member of an interdisciplinary team of health care providersSalary/Benefits: Salary commensurate with experienceSee website www.partnershomecare.org Job Location: Waltham, MAJob Number: 3083Company URL: http://www.partnershomecare.org/
