Medical Social Worker

Provide wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and Pre-Bereavement/anticipatory grief needs. Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Regularly re-evaluates the patient’s and family’s psychosocial needs. Adheres to Company policies and procedures and regulatory compliance requirements and directives.    REQUIREMENTS 1. Minimum age requirement of 18. 2. Bachelor’s degree in Social Work, Master’s degree preferred. Masters degree from school accredited by the Council on Social Work Education required in states as specified. 3. LBSW or LMSW required. 4. Minimum of two year’s social service experience in a health care setting or social services agency. 5. Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). 6. Compliance with accepted professional standards and practices. 7. Understands Hospice philosophy, principles of death/dying. 8. Ability to work in an interdisciplinary setting. 9. Physical exam in accordance with state law requirements. 10. Valid driver’s license and an automobile that is insured in accordance with state and/or Odyssey requirements and is in good working order. 11. Satisfactory references from employers and/or professional peers. 12. Satisfactory criminal background check. 13. Compliance with accepted professional standards and practices. 14. Ability to work within an interdisciplinary setting. 15. Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner. 16.
