Join our beautiful Scottsdale location, located adjacent to the Scottsdale Fashion Square Mall! Health Coach (Master’s Social Work) This individual is responsible for providing telephonic wellness health coaching to participants enrolled in the Wellness Coach Programs. This includes conducting assessments, providing education and assisting individuals in managing their health behaviors. KEY RESPONSIBILITIES Engages program participants in modifying high risk lifestyle behaviors (i.e.; overweight, high stress, low activity, smoking) Assists program participants in developing and or maintaining self care plans through utilization of goal setting strategies. Uses clinical judgment to make appropriate referrals for program participants Identifies, educates, facilitates and evaluates attainment of self-management goals. EDUCATIONMaster’s degree in Mental Health field required. AZ licensure as a LCSW, LMSW, LPAC or LPC preferred.Prefer individual who has experience working with individuals who have chronic medical conditions. Minimum three years experience.
