Join us, do rewarding work, and build a great career.   Considerable recent growth has highlighted our need to fill multiple openings for Master Social Workers in Knoxville, TN.  The primary goal of the Master Social Worker is to provide support to assigned Care Managers in coordinating and facilitating services identified in the member plan of care.    Major responsibilities include:  Assessment: gathers in-depth, comprehensive information about member’s situation, functional ability, resources and barriers, and cognitive status from appropriate resources which may include: family members, spouse, caregivers, and providers. Planning: develops a plan of care with goals that are specific, measurable, achievable, realistic and time specific to member needs. Education: of the member and family/ caregivers on self-care management, empowering to make decisions about care and appropriate utilization, to insure best outcomes for the individual. Monitoring: provides care coordination on an ongoing basis to determine effectiveness of the plan of care. Assess clinical information at established intervals to evaluate the effectiveness of the case management plan for each member. Assess all cases for potential complications, appropriateness of care, and recommendations for alternative care if indicated Evaluation:  provides regular follow-up and monitoring of services and client condition with the intention of updating or modifying the plan of care based on changing member needs.  The requirements for this role are as follows:  Current, unrestricted MSW license in the state of TN 3 + years Social work experience in a healthcare environment PC proficiency to include MS Word and the ability to talk and type at the same time Case management experience    Additional assets include:Medicare / Medicaid (TennCare) knowledge/experience Ability to assess, prioritize, and address member needs through structured and focused interventions Ability to use community and network resources Communication and motivation skills Excellent public and community relations skills Excellent problem solving skills Strong documentation skillsWhen you become part of AmeriChoice, you also become part of something larger—a Fortune 100 leader with a broad spectrum of services and products—while still working for a company where you can see and feel your impact.
