The true heart of this pursuit of excellence can be found in our Comprehensive Care philosophy, in our employees, and in our mission to meet the physical, emotional, and spiritual needs of all eligible patients. www.VistaCare.com Leading Hospice Provider Seeks Licensed Master Social Worker Licensed Master Social Worker: *When requested by the patient or family you will be responsible for assessing the psychosocial, community service or economic needs of our patients and their families. *Assist in developing a Plan of Care to meet identified needs. *You will also, be the key contact in assisting the patient and family in the development of the Advance Directives. *Provides short term counseling services *Maintains knowledge of state Medicaid requirement, Advance Directives, POA/DPOA issues and any changes that occur. Requirements QUALIFICATIONS: * 2-5 years experience as a Social Worker. Previous hospice or home care experience helpful. * Master's degree in Social Work. Must be licensed through the State of Texas. * Experience with local community resources. * Experience working with the elderly. * Excellent communication, time management and customer service skills. * Reliable transportation, current driver's license and current auto insurance
