As the Manager of Clinical Practice with Gentiva, you will: *Conduct or delegate the assessment and reassessment of patients, including updating care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures and policies. Manage the assignment of clinical associates. *Coordinate communication between team members and attending physicians or clinical associates to ensure the appropriateness of care and outcome planning. Ensure compliance with third party requirements through review of documentation and care coordination activities. *Interpret standards, company policies, and procedures to ensure compliance with external regulatory authorities and ensure that clinical associate documentation meets internal standards. *Consult with third party representatives regarding patient financial responsibility, service authorization, and reimbursement procedures. Negotiate reimbursement levels and facilitate the process. *Participate in the recruitment/interviewing/selection/orientation of team members, and evaluate their performance relative to job goals and requirements. Coach staff, recommend in-service education programs, and ensure adherence to internal policies and standards. Qualifications * Baccalaureate degree and at least 3 years of full time clinical experience in nursing with in the past 5 years of at least 1 being in a home health office or community health program which includes care of the sick at home or a diploma in Nursing / associate degree in nursing with the required experience above plus either certification by ANA s a community health nurse or 6 credits in health care management from an accredited college or university program or school of nursing. *Meet any additional qualifications as stated in state home care regulations *Minimum of three years experience in area of specialty preferably in a community or home care setting *One year in a supervisory role generally required *Licensure in the state(s) of practice *Excellent organization, problem-solving, and communications skills Benefits We recognize people as our most valuable asset.
