Company Overview
St. Mary Medical Center is sponsoring a nationally recognized Program of All-inclusive Care for the Elderly (PACE), a new federal program for older adults. There are currently about 50 PACE programs in the country.
Called LIFE St. Mary, the program is an innovative model of care that offers older adults the option of remaining at home with dignity and independence. The program is based on the belief that seniors with chronic care needs and their families have better quality of life and health outcomes when they receive services in the community.
LIFE will serve individuals who are: aged 55 or over, assessed as needing nursing home-level care, able to live safely in the community with LIFE services at the time of their enrollment, and eligible for Medicaid or able to pay privately.
A hallmark of the PACE model is the interdisciplinary team, which develops plans of care encompassing preventive, restorative, maintenance, and palliative care, depending on the participants' needs and wishes. The focal point of care is the LIFE Center, where participants receive many of the services offered by LIFE. Services include primary medical care, nursing, health education and wellness, dental and vision services, medications, rehabilitative therapies, social services, nutrition services (including nutritious meals), and much more.
LIFE St. Mary will open the first LIFE Center in Lower Bucks County, and additional centers in other areas of the county in the near future.

Job Description
Responsible for developing the annual marketing plan and undertaking planned activities to meet stated census targets. Success in this position depends on developing referral relationships and sustaining referrals to LIFE St. Mary, a Program of All-inclusive Care for the Elderly (PACE), as a result of satisfied referring professionals. You will supervise other enrollment staff and also serve as a front-line marketer and enrollment specialist with potential participants and their families.
