Manager, Disease Management Program (08-48)
HealthPlus of Michigan has an excellent opportunity for an individual to join our leading health insurance organization. Responsibilities for this position will include ability to develop and implement Plan-wide disease state management programs for chronic conditions. The successful candidate will have a Bachelor’s degree in nursing. Also, a current, valid Michigan license to practice as a Registered Nurse without any conditions, limitations or restrictions, two (2) years of management experience, five (5) years clinical experience in the care, treatment and management of patients with chronic diseases and three (3) years experience in population-based health care.
