MENTAL HEALTH CASE MANAGERS

PURPOSE: To provide case management services for consumers with chronic mental illness.

 

ESSENTIAL FUNCTIONS / JOB RESULTS

·  Provide case management services to assigned clients to include treatment planning, community outreach, and counseling as needed.Establish appropriate and therapeutic relationships with assigned clients.Provide individual, group, or family counseling and/or education about mental illness as needed.Maintain all required paperwork / charting requirements as established by agency contracts.

Assist with meeting basic needs by:Linking to funding sources, assisting with obtaining and maintaining income and benefits, teaching and/or supporting ability to mange finances, linking and working with payee.Assisting in finding and obtaining resources in emergency situations.Helping with resources for housing and shelter, assisting with finding resources for security deposits, etc, enhancing ability to maintain housing, helping to locate and facilitate supported/assisted living or community resources for in home care.Conflict resolution/mediation for disputes with landlords, roommates, neighbors.Teaching housekeeping, cooking and shopping skills, helping to arrange for housekeeping supports.Assisting with locating and learning how to use available transportation.  Other duties as assigned by the Supervisor.

 

MINIMUM QUALIFICATIONS

CASE MANAGER I:  Bachelor’s degree in the human service or related field.  At least one year’s experience working with adult mental-health consumers.

 

CASE MANAGER II:  Master’s degree (or current enrollment in a Master’s program and due to receive degree within six months of hire) and two years experience with mental- health consumers.

 

ALL:  Must possess current valid Washington State Registered Counselor credential.  Experience with specialized SPMI populations desirable.  Good communication and interpersonal skills, basic knowledge of chronic mental illness, customer service, maintaining and building therapeutic relationships.  Assessment skills, multitasking, making decisions, problem solving, intermediate computer literate, and good time management skills.  Flexible and open to change.  Knowledge of local housing resources.  Driver’s license, automobile liability insurance, and reliable transportation.  Research skills.
