Long Term Care Pharmacist

POSITION SUMMARY: Prepares and dispenses medication orders per physician request according to established policies, procedures and protocols.  Position prepares sterile products and assumes responsibility for patient outcomes.  Evaluates pharmaco-economics.  Contributes to the quality and effective operation of the department.  Maintains competence required for current job title/position.EDUCATION: Graduate of ACPE accredited School of Pharmacy with a BS Pharmacy degree. National Certification as Diabetes Educator or Geriatric Pharmacist highly encouraged.  Licensed (or immediately eligible) as Registered Pharmacist in designated state. EXPERIENCE: 2 years pharmacy experience in long-term care or hospital preferred.  SKILLS: Ability to work independently with minimal direct supervision; meet pharmaceutical care needs of adolescent, adult, geriatric, and skilled nursing patients; make critical decisions and respond quickly to customer needs; and manage diverse workforce.  Strong organizational and communication skills.  Computer proficiency in word processing, spreadsheets and data entry.
