Unlike for-profit home care and hospice agencies, we do not report to shareholders. Instead, all of our revenue is used to provide patient care and to support our policy to provide care regardless of a patient's financial circumstances.   Sutter VNA & Hospice is committed to being the "employer of choice" through competitive wages and benefits, flexible schedules, new technologies including laptop computerized charting, tuition reimbursement, and more!   Under the general direction of the Clinical Supervisor, the Licensed Vocational Nurse assists in the nursing process by instructing patients and care givers in self care and providing basic nursing care as needed.  Education: Graduate from an accredited vocational school of nursing or equivalent.  Licensure: Current California LVN license.  Must possess current BCLS/CPR certification. Experience: Minimum 1 year of professional experience strongly preferred.  Home health or community health experience preferred. Knowledge of working with community resources and skilled nursing facilities. Knowledge: Basic competencies in the fundamentals of nursing and pharmacology are required. Special Skills/Equipment: Demonstrated skills in effective written and verbal communication.  Ability to recognize the needs and concerns of diverse groups of people.  Ability to maintain harmonious constructive working relationships with internal and external customers (patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources).  Ability to use and give instruction of adaptive equipment.  Must possess a current California drivers license and access to a reliable automobile with proof of liability insurance. Physical Requirements:  Essential job duties and physical demands include and are not limited to: Administers skilled nursing patient care in a home or assisted living environment; following written or oral instructions; assists patients in walking and transfer from bed to chair and in patient positioning. Must be able to remain sedentary for approximately 50% of the day, maneuver around objects, climb stairs and/or work in relatively small spaces, bend over or stretch as necessary in care provision, operate telephone and manipulate small clerical items (e.g. paperclips, staples, etc.); ability to raise, lower, or transfer objects and supplies from one level to another; transport an object usually by hands, arms or shoulders; ability to bend, stoop, twist and lift and transfer up to 50 pounds in patient care.  Must be able to meet company productivity standards as indicated by organization and manager.  Must be able to hear, read, write and speak clearly to effectively communicate with department staff, other Sutter VNA & Hospice employees and community, and carryout essential functions.  Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter VNA and Hospice's strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor.  Must be able to deal with challenging work environment with time demands and occasional conflicting priorities.  Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.  Is responsible for maintaining all required licensure and certifications.  At orientation and annually thereafter, must be able to meet Sutter VNA & Hospice core competencies for position and area of specialty, as appropriate.  Must have reliable transportation.
