Licensed Social Worker Case Manager

Seeking Licensed Social Worker Case Manager to work with leading health and wellness organization.  Under administrative and clinical direction from the Case Management Team Leader provides timely, clinically appropriate and cost effective case management interventions with focused utilization management activities. Care Management interventions are based on nationally recognized standards of case management practice. The Case Manager will develop effective working relationships with members, families and providers in order to: actively participate in the assessment of the member’s needs and the development of individualized care plans, matching the available Health Plan and community services to those needs; coordinate the ongoing evaluation of the care plan; recommend and facilitate adjustments to the care plan and services in place; focus on optimizing the member’s/families level of independence in navigating the healthcare system at all levels of the continuum; and utilize opportunities to educate them.  Their case management interventions are performed telephonically. 

Education: Licensed Clinical Social Worker required.  CCM certification preferred but not required. Previous case management experience is desired.
