LVN/Intake Coordinator, Garland TX
Home Health Services is a nonprofit, community-based home care agency that is accredited by JCAHO and certified by Medicare. Home Health Services has been meeting the needs for home health care specialized in serving the senior population since 1974.
We are currently seeking qualified candidates for a Full Time LVN-Intake Coordinator/ Scheduler position at our Garland, TX office.
The Intake Coordinator may be a Registered Nurse, Licensed Vocational Nurse (LVN), who under the Branch Manager, provides coordination prior to initiation of home care and on an ongoing basis. In addition, the Intake Coordinator facilitates communication among physicians, discharge planners, clinical staff, and patients.
Qualifications
§ Minimum of one (1) year experience in home health care or as a discharge planner in the hospital.
§ General office and computer use skills. Must be proficient with MS Windows software packages, email, use of fax machine, and have strong office skills.
§ Excellent organizational skills, good judgment, and communication skills.
§ Current Texas License as a Registered Nurse or Licensed Vocational Nurse is required
§ Complies with accepted standards and practice
§ Is self directed with the ability to work with little supervision.
§ Is flexible and cooperative in fulfilling all obligations. Able to accept constructive criticism and use it in a positive manner.
Responsibilities and Duties
§ Receives the initial referral and confers with the referral source regarding coverage criteria and appropriate agency services.
§ Coordinates the home care needs of the patient with the agency in accordance with the physician’s plan of care requirements.
§ Communicates with the patient/family to acknowledge receipt of referral and makes arrangements for the initial admit visit.
§ Coordinates with community agencies, i.e.: DME, pharmacy, and discharge planners, to arrange for equipment, supplies, and medication prior to patient’s admission.
§ Manages referrals so that the initial visit occurs within 24 hours. Documents circumstances that precludes admission within 24 hours.
§ Initiates appropriate verification forms for Medicaid, Private Insurance, Sliding Scale/Non-Funded and other special funding referrals.
§ Notifies the Billing Manager that a non-Medicare referral has been received and the appropriate verification forms have been completed.
§ Ensures that the written verification for home care services is attached to the RIS prior to the initial visit.
§ Keeps current data on staff activity and availability to ensure adequate coverage of patient visits.
§ Assigns clinicians to patient cases according to the clinician’s qualifications/availability and the patient’s needs/acuity.
§ Communicates with the appropriate team manager and field clinician regarding pertinent patient information, i.e.: problems, condition changes, physician orders, lab results, scheduling or coverage problems.
§ Acts as liaison between clinical staff and physicians when needed to facilitate communications about patient condition and/or care needs.
§ Monitors hospitalized patients’ status on a daily basis and pending discharge home care needs to ensure continuity of care.
§ Notifies the onsite case managers, discharge planners or others when patients are readmitted to a health care facility to maintain continuity of care.
§ Notifies appropriate agency staff when patients are readmitted to a health care facility or have expired.
§ Notifies appropriate agency staff and schedules patients for a post hospital assessment visit within agency timetables. Completes post hospital referral forms with pertinent information and makes assignments to the appropriate clinician(s).
§ Maintain documentation/logs pertaining to new referrals, discharge/transfer, deaths, hospitalizations, patient assignments, rejected referrals.
§ Coordinates on-call/weekend and other unassigned visits to ensure adequate coverage by obtaining patient report forms for each visit.
§ Utilizes effective follow-up to verify that each visit has been assigned and that the clinician has been notified and the patient care report is given.
