The Field Licensed Practical/Vocational Nurse is responsible for implementing care plan and educating patients/caregivers. Demonstrates competency in patient care standards required for safe delivery of services as evidenced by completion of orientation and licensed practical/vocational nursing skills checklist.Will be competent in • the basic nursing skills, • basic infusion skills, • basic wound care, and • completing clinical documentation. Manages Time effectively to deliver accurate and appropriate patient care. Arranges daily activities with regards to patient safety and team communication. Under the direction of the Registered Nurse, implements the Plan of Care. Effectively communicates with agency staff, medical team, patients, and family. Under the direction of the registered nurse and in accordance with the plan of care, safely performs and documents nursing care procedures in a manner consistent with scientific principles and agency policy.Prepares and submits documentation outlining services rendered within the required time frame. Communicates with registered nurse and supervisor regarding problems, change in status, and goal attainment. Participates in multidisciplinary team conferencesMaintains Confidentiality of patient and proprietary informationObserve legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information.Qualifications:Minimum Requirements:• Currently licensed by the State Board of Nursing Examiners, in the State of practice.• Has at least one year experience as a LPN/LVN and shall work under the supervision of a registered nurse.• Satisfactory completion of a pharmacology course or its equivalent.• Flexibility and the ability to work effectively within a multidisciplinary team• Ability to communicate effectively (verbal and oral) with team members, patients, caregivers, physicians and referral sources Preferred Qualifications:• Previous Home Health Care Experience• Satisfactory completion of Infusion Therapy course work• Previous Infusion Experience.
