Receive and document medical orders, accept for services if appropriate and fully process (or delegate) referrals in an accurate and timely manner. Communicate pertinent information to involved parties according to Intake and PHC guidelines.Qualifications:Hours needed for this position: Monday -Wednesday 8:00 am -4:30 pm & Thursday and Friday 9:30 am- 6:00 pm*Professional LPN license to practice in Massachusetts*1-2 years experience in a community home health, liaison, discharge planning or managed care environment, proficient computer skills and understanding of insurances. *LPN license to practice in Massachusetts*2-3 years general medical-surgical experience in an acute setting*Basic computer skills with ability to learn new software applications*Highly developed communication and organization skillsSalary/Benefits: Salary commensurate with experienceSee website www.partnershomecare.org Job Location: Waltham, MAJob Number: 3278Company URL: http://www.partnershomecare.org/
