Inpatient Case Manager
POSITION: Inpatient Case Manager
' EXEMPT
REPORTS TO: Inpatient Case Management Supervisor
POSITION SUMMARY:
Mount Auburn Hospital is a 190 bed acute care teaching hospital affiliated with Harvard Medical School. We currently have 2 full time positions open for Inpatient Case Managers. The Inpatient Case Manager (CM) performs utilization review and coordinates discharge planning for MAH inpatient admissions. Inpatient Case Manager performance is evaluated using the Inpatient Case Manager Guidelines and audit tool.
PRIMARY RESPONSIBILITIES:
1. Using established clinical criteria, identifies days not meeting acute hospital level of care criteria and takes appropriate action.
2. Collects data on hospital days not meeting acute criteria and forwards to supervisor for
review at hospital Utilization Management Committee meetings.
3. Works with staff, patients and families to develop plans to meet short and long-term goals,
and to facilitate post-discharge needs.
4. Acts as the liaison between post-acute providers and the patient.
5. Is part of the department’s Saturday, holiday and beeper rotation as needed.
6. Adheres to all insurance, contractual, and regulatory requirements as mandated.
7. Abides by MACIPA Case Management standards as outlined in the MACIPA CM guidelines.
8. Other duties as assigned.
SKILLS/KNOWLEDGE AND ABILITIES
1. Bachelor's degree preferred. Current unrestricted Ma. state license as a Registered Nurse required.
2. Three years of recent medical/surgical nursing care experience required. Two years of case management or utilization management experience desirable.
3. Basic computer and keyboarding skills.
4. Effective communication, problem solving, and conflict resolution skills.
5. Demonstrated ability to organize and work independently.
