Hospice RN Case Manager
Work with a great team
In-House Hospice providing quality hospice care for over 10 years has a full time position for a Registered Nurse Case Manager. Current license required. Hospice experience preferred. Provide assessment, care plan oversight, support and care to hospice patients and caregivers. Patients residing in Long Term Care settings, home settings, and Assisted Living. Work with an interdisciplinary team.
