Licensing in Missouri is required Illinois not required but a plus. LPN and/or RN license required for position. Position will consist of conducting start of care visits, routine check-ins, nursing assessments, and discharge visits. While also conducting the visits nurse will be responsible for case management as well. Additional duties while on the visit would include administering and monitoring all medications; monitoring all indwelling lines and life support devices. Provide health teaching and support, as well as assess vital signs.
