These positions, focused on telephonic case management and patient/provider education, offer plenty of career growth, job variety and a reliable M-F weekday schedule. General responsibilities of a Healthy First Steps RN Case Manager include: Managing cases for normal and high-risk Obstetric patients in the TennCare program Making outbound calls to patients to discuss their health risks, manage compliance and educate them on pregnancy-related issues Working with Providers to drive patient compliance and help the providers with referrals Running daily queries to determine appropriate members to be contacted Documenting all case management activities in our internal system The requirements for this role are as follows:Current, unrestricted RN license in the state of TN 5 + years high-risk pregnancy experience in a clinic, physician office, hospital or public health agency setting 2 + years case management experience PC proficiency, to include MS Word and the ability to type and talk at the same time; for future telecommuting opportunities, hired candidates MUST have DSL capabilities in their home Demonstrated ability to work independently and manage own caseloads Demonstrated ability to work with patients and physicians via phoneAdditional assets include:Medicare / Medicaid (TennCare) knowledge/experience Knowledge of Milliman criteria Ability to adapt to a fast-paced, ever-changing environment Ability to work with a diverse member population CCM designationWhen you become part of AmeriChoice, you also become part of something larger—a Fortune 100 leader with a broad spectrum of services and products—while still working for a company where you can see and feel your impact.
