Health Coach- RN / 732 Position Description:RN - Health CoachDescription:Responsible for provision of education and support to members to assist in learning to self-management their chronic illness.Responsibilities:Health Coaches are responsible for providing disease management, training, and other clinically based activities as assigned to coordinate care between providers, the member and the community. Communicating with providers in the community regarding the consumer's treatment needs and developing appropriate care plans for specialized needs of members. Responsible for medical record reviews and giving feedback to Primary Care Physicians regarding their obligations under the DCH program Performing telephonic case management to empower members to better understand their illness and self-manage their conditionAs appropriate, provide group education to members in community location. Education is specific to disease processes, nutrition, wellness, etcWill make adequate contact, to members, as stated in Program guidelines with a focus on behavioral goal setting and member centered support planningWill verbalize/demonstrate understanding of diabetes, asthma, COPD, CAD, and CHF disease processes and self care managementWill identify member resource needs and provide information on possible resources to members Will make referrals for members, as applicableTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education:Registered Nurse with active license in Georgia. Requirements:Minimum of 2 to 3 years experience in related public sector medical services including experience with quality improvement, medical record reviews, disease management and/or primary care case managementExperience with chronic illness and disease states targeted by DCH strongly preferredStrong verbal and written communication skills Customer service focus; and ability to resolve problems in a non-threatening manner.
