Yoh Health Care has a direct hire opportunity for a Geriatric Health Manager to join our customer in Long Beach, CA.Purpose: To conduct telephonic care management assessments of members, classify members as needing either basic or complex care management, developing appropriate care plans and advocating on behalf of the members as needed. Responsibilities: • Conduct telephonic assessments and reassessments at designated intervals to identify member‘s medical, physical and psychosocial needs develop written care plans and act on those care plans in a timely manner. • Create and maintain clinical documentation of each interaction with the member and/or on the member‘s behalf. • Facilitate care coordination across the care continuum: home, hospital, home health, or nursing facility. Identify necessary community resources and make referrals as appropriate. Interpret plan benefits by clarifying and verifying the organization‘s policies and procedures and staying abreast of coverage issues for the Medicare and MediCal programs. • Participate in an Inter Disciplinary team by presenting member scenarios to the team for discussion, at least annually. • Coordinate purchase of service, by identifying appropriate vendors, seeking competitive pricing bids and allocating resources in a cost-effective manner. • Maintains the member‘s right to privacy and protects operations by keeping information confidential. Requirements: • Graduate of an accredited school of nursing or school of social work. • Minimum current licensure in the State of California as a RN, or MSW degree from an accredited school of social work. • Bachelors or advanced degree preferred • with a minimum of two to three years experience in medical or social carse with a geriatric population. • Case management certification highly desirable. • Good oral and written communication skills, with the ability to speak clearly and conduct telephonic assessments. • Bilingual (Spanish) a plus. • Computer literacy required with competency in MS Word, Excel and Access. • Demonstrate ability to work with minimal supervision. Education: • Graduate of an accredited school of nursing or school of social work.
