Entitlement Coordinator
Job Description: Entitlement Coordinator/IAC Case ManagerThe entitlement coordinator plays an integral part in providing intensive case management services to assist IAC patients and their families. Case Managers are teamed with a Social Worker and Parent Advocate at the start of an IAC patient's treatment to assess the needs of the patient and the family unit. The case manager will engage in service planning, monitoring and strengthening and empowering the family unit. In addition, the case manager will provide and maintain a network of formal and informal support services that will meet the multiple needs unique to each IAC patient and their family. There are three main components: Community Linkage, Team Collaboration and Programming. Community Linkage * To conduct case management assessment * To conduct home visits and engagement with the family unit * Establish an educational plan and arrange for primary long-term medical care (i.e. providing a secure educational environment, collaboration with teachers and other professionals, schedule planning, special education programs for full transition etc) * Establish rapport with recreational facilities, community supports including substance abuse treatment programs, legal services, housing and immigration * To maintain the lines of communication with outside agencies * To serve as an escort to patients and their families (i.e. doctor appointment, school visits, etc) * To follow-up on securing eligible entitlements (i.e. Medicaid, childcare, housing)* To schedule and monitor patient's appointments * Provide appropriate letters of referrals/recommendations to agencies and schools Team Collaboration * Assist the Social Worker in the treatment plan process of patients * Co-facilitate parent support groups with a Parent Advocate or Social Worker (i.e. parenting skill training) * Co-facilitate an assist colleagues on service planning to ensure for patients smooth transition * Provide direct task supervision to Parent Advocates (i.e. escorting, scheduling and follow-up, support groups) Programming * Co-facilitate in the development of program infrastructure and documentation * Participate in case conferences and team meeting (i.e. presentation of cases, feedback, new ideas) * Participate in mandatory staff training and departmental conferences (i.e. safety issues, emergencies, Grand Rounds both Child Psychiatry and Social Work) * Manage and monitor funds that will be provide as additional support to families * Update and maintain resource manual & directory * To conduct visits to area agencies and build on resources Job Qualifications: Bacherlor's DegreeBilingual SpanishKnowledge of Entitlements for Medicaid and Medicare Mount Sinai Medical Center is an equal opportunity/affirmative action employer.
