Make a healthy choice for your career and apply today.   Primary Responsibilities:   Under the direction of the Quality Manager the Discharge Specialist will follow up on the treatment of members being discharged form acute care, while focusing on the following key responsibilities:Promoting the health and well being of high risk members Speaking with members to ensure compliance with identified treatment plans Clarifying and confirming discharge plan details with the members Calling providers to arrange for follow up appointments if necessary Identifying situations that need to be referred to the Care Managers intervention Collaborating with other clinical staff to enhance the quality and efficiency of health care delivery Educating members on discharge processes and documentationQualifications:1+ years Healthcare experience (working in a doctor's office, hospital, health department or clinic) is required. 6 months + Medical terminology required. 1+ year experience with MS/Word & Excel. HS Graduate or equivalent is required Experience in a customer service or telephonic role is preferred. Knowledge of community based resources and services are preferred. MS/Outlook experience is preferred.
