Director of Care Coordination

Position Summary:

Responsible for the performance of a multi-disciplinary Care Coordination Department.  Establishes goals for the department and oversee its development and performance.  Responsible for the development of short and long term planning and goal setting. Develops new programs and expands existing services. Establishes systems that ensures continuity of care.


Position Qualifications:

Masters degree Nursing or equivalent experience with five years of supervisory experience in hospital or healthcare environment required.  Current Massachusetts RN license and registration.  Minimum two years experience in Case Management.  Knowledge of Utilization Review and Interqual Criteria. Demonstrated ability to plan, organize and evaluate programs and to analyze fiscal data and develop departmental budgets. Excellent written and oral communication skills. Knowledgeable and skilled in the use of personal computer software. In addition the Director should have experience and knowledge in the area of Utilization review.
