DM Coordinator / 809 Position Description:DISEASE MANAGEMENT COORDIATOR/LPNAPS Healthcare, an innovator in the delivery of healthcare services to state and local governments, provides an array of award winning health and disease management programs. Working collaboratively with government leaders, we implement cost-effective, high quality programs that improve overall beneficiary health and enhance provider support. We are looking for dynamic LPN's to help pave the way for Medicaid reform in our Missouri program. As part of the Missouri team you will be involved in one of the largest Medicaid Disease Management programs in the country. The DM Coordinator is an integral part of the Health Services team involved in supporting Medicaid members to improve their health status.Responsibilities:• Communicate with members telephonically to introduce the disease management program and explain its benefits;• Gather demographic information and may gather general health information from members;• Coordinate follow up appointments between nurses and members for completion of health assessment and care planning;• Distribution of program introductory materials and member educational materials;• May provide educational, nutritional and/or disease specific information, based on education and experienceEducation:• LPN licensed in the state of Missouri• Associate or bachelor's degree in health field preferredRequirements:• Minimum 4 years customer service experience.• Healthcare experience required.• Experience with chronic illness and disease states preferred.• Strong verbal and written communication skills.• Ability to resolve problems in a non-threatening manner.• Experience in a call center environment and data entry, preferred. • Previous experience in delivering care management services to the Medicaid population preferred.• Above average computer/technical skills.• Ability and experience working with a diverse population.Our company is committed to maintaining a challenging environment that promotes personal accountability, personal growth, and an active role in the driving vision of the company.
