Coordinator Clinical Excellence: Cardiac and Stroke
The Coordinator reviews complex medical records and other documents to obtain data for cardiac and stroke performance measurement. Extracts data for analysis and interpretation for action based improvement in clinical outcomes using multiple databases, such as ACC, STS, Stroke-GWTG and other decision support systems. Serves as a facilitator providing development and implementation support for clinical quality improvement and disease management teams, focused on improving the care and outcomes experienced by patients with select medical conditions including but not limited to chest pain, AMI, CHF and CVA. Guides individual patient case and patient population focused medical department and interdisciplinary team review with development of process improvements. Serves as a catalyst ensuring that evidence based practice is achieved, PI targets met and adverse events minimized. Qualifications: Bachelor of Science degree in Nursing or a related field preferred. Minimum of five years experience in a health care setting in patient care. Minimum three years demonstrable experience working with computers and windows based software, particularly Microsoft Office applications. Ability to create complex reports using SPSS. Strong interpersonal skills, project management expertise, expertise in data management Experience in disease management, care management, outcome reporting and physician profiling Ability to work independently with minimal supervision. Must have excellent written and oral communication skills.
