Clinical Pharmacist I/II - Inpatient (RF-Night)
Exempt/Gr. TBD - Salary is commensurate with experience, skills, etc.BASIC JOB PURPOSEUnder general supervision, PROVIDES pharmaceutical care by performing various patient focused clinical, educational, informative, and distributive functions in inpatient and/or home infusion as interdisciplinary team member in support of patients, providers, and ancillary staff; ENSURES appropriate, cost-effective drug utilization while maintaining and enhancing patient care; SUPPORTS compliance and Principles of Responsibility; MAINTAINS confidentiality; PROTECTS organizational assets; EXHIBITS ethics and integrity; ADHERES to applicable federal and state laws and regulations, accreditation and licensing requirements, policies and procedures; REPORTS and/or RESOLVES issues of non-compliance.On a regular, sustained basis, COOPERATES with other staff members both within and outside the department in accomplishment of own job duties as well as assisting others in accomplishing theirs. SERVES as team player and role model for other employees in the organization; always exhibiting traits of courtesy, caring, helpfulness and respect. CONDUCTS self in a service-oriented manner that is attentive, pleasant, cooperative, sensitive, respectful and kind when dealing with members, visitors, the public and all employees.MINIMUM QUALIFICATIONSEDUCATION/TRAININGGraduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE).Completion of an emergency contraception training course approved by the American Council of Pharmaceutical Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy, applicable state or local health department programs, or programs recognized by the board of pharmacy. Equivalent combination of education & experience may be considered in lieu of requirements.LICENSURE/CERTIFICATIONValid Hawaii Pharmacist license (must meet education requirement(s) for Hawaii State licensure); Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.EXPERIENCE/OTHER PHARM CLIN II - INPTTwo years full-time practical (post graduate) experience or completion of one year general residency; Demonstrated knowledge of related federal, state and local regulations and accreditation requirements including JCAHO standards.PHARM CLIN I/II - INPTDemonstrated ability to safely handle and/or prepare various pharmaceutical compounds (e.g., chemotherapeutic agents, biologicals); Demonstrated knowledge of and skill in the following: adaptability, change management, conflict resolution, customer service, decision making, interpersonal relations, oral communication, problem solving, quality management, teamwork and written communication; Demonstrated knowledge of and skill in the following PC applications: word processing, spreadsheet and database.Equivalent combination of education & experience (lesser) may be considered in lieu of requirements.COMMUNICATION AND MENTAL REQUIREMENTSTalking to co-workers, customers, outside vendors, and on the telephone; Reading, writing, speaking and understanding English; Training/giving and receiving instructions; Mathematical ability; attention to detail (e.g. organization, prioritization, proofing); concentration and alertness.PREFERRED QUALIFICATIONSEDUCATION/TRAININGDoctor of Pharmacy Degree from a school recognized by the American Council of Pharmaceutical Education (ACPE); Completion of a general residency program.LICENSURE/CERTIFICATIONBoard Certified Pharmacotherapy Specialist (BCPS) by the Board of Pharmaceutical Specialties (BPS).NOTE:- Work schedule and days off may be changed based on operational needs.- Should you submit your resume for this position and you do not meet the minimum qualifications, or are not contacted within 2-6 weeks you should assume that a more qualified candidate was selected or is being considered for the position.
