Outreach Project is a non-profit agency which has been providing quality drug and alcohol treatment services in the NY metropolitan area since 1980.  Outreach Project's mission is "To empower individuals, families and communities to make choices which will enable them to experience a responsible life free from alcohol and drug abuse".Outreach Project currently operates programs in Queens, Brooklyn and Long Island.  Please visit our website:  www.opiny.org for additional information on our program services.We are currently seeking qualified candidates for the position of Case Manager in the TANF (Temporary Assistance for Needy Families) Track of our outpatient substance abuse treatment program in Bellport, Long Island.The principle duties and responsibilities include, but are not limited to: Assessment and referral to enhance participation and improve employment (SWEP) outcomes for TANF clients by educating clients regarding rights and responsibilities in relation to DASAS, DSS, DOE. Case management, crisis intervention and home visits to assist in the coordination of treatment services in line with workforce initiatives and promote access to transitional support services and clients dealing with sanctions, barriers to employment and self-sufficiency. Determination of eligibility and completion of appropriate paperwork; coordination with treatment team in the development of an individualized treatment plan. Referring and advocating for services for the client and/or family members. Interfacing with appropriate agencies and systems (DSS, probation, courts, etc.) on behalf of the client and assisting clients in meeting all mandates. Ongoing case management related to housing, transportation, childcare, food and other related needs. Intake assessment of new admissions and case management services for TANF clients. Maintain communication and treatment planning at Bellport and direct service providers and Suffolk county vocational service providers. Submit weekly schedules, ensure prior approval for off-site activities, monitor and enhance effective utilization of time. Attend and participate in staff/community meeting where appropriate. Provide crisis intervention and group work services and back up coverage, as needed. Maintain computer skills that are appropriate to the level needed for optimal job performance.
