Case Manager (Supportive Housing)
The Supportive Housing program provides case management services to
Burlington County residents who have a serious and persistent mental illness and
who possess the desire and ability to live and participate independently in their
community.  Case Manager position provides crisis intervention, money
management assistance, treatment planning, recreation, life skills training,
consultation, advocacy, and referrals to community services. Facilitates the
transition of individuals to independent housing. Provides care for consumers
individually and as a group, in addition to other duties as required.Â
Position required Bachelor’s degree in a mental health, social work, or related
field. Two years case management experience required. Experience with MICA
Â
consumers desired. Rotating on-call responsibilities.
