Case Manager (RN)

JOB DESCRIPTION

 

The Certified Case Manager (R.N.) is responsible for the coordination of care and services between employees, employers and carriers, as well as providing a full range of nursing- and health-related activities that ensure the clinical effectiveness and excellence of the Viverae Delivery Department.  Working closely with the Director of Clinical Services, the Certified Case Manager (R.N.) will help develop clinical health and wellness programs, participate in telephonic and secure message coaching, and conduct worksite health screenings and presentations. 

 

QUALIFICATIONS

 

·        Degree from an accredited nursing school required, Bachelor of Science in nursing preferred

·        Current unrestricted registered nurse (R.N.) in the state of Texas.

·        3+ years clinical nurse experience with a preference for previous insurance nurse case management.

·        Preferably holds the following certifications:  CCM (Certified Case Manager).

·        Excellent verbal and written communication skills.

·        Experience in health promotion, public health or wellness is preferred.

·        Proficient computer skills, including Microsoft Office usage and Internet searches.

·        Bi-lingual in Spanish is desirable.

 

SUMMARY OF KEY RESPONSIBILITIES

 

·        Conduct coaching interactions – in person, via phone, through secure messaging and by email.

·        Develop clinical policies and procedures.

·        Deliver health and wellness programs to clients – in person or via the web.

·        Perform a variety of on-site health and screening assessments (as needed).

·        Manage members’ chronic health conditions and preventive health compliance.

·        Must be able and willing to travel to clients throughout the United States (on a very limited basis).

·        Other duties as assigned.
