Case Manager/Program Director Needed for an Adult Day Health Care

Program Director or Nurse Manager needed for an Adult Day Health Care in Anaheim, CA



RN or medical background preferred.  Candidate must possess strong leadership qualities that include but are not limited to management, organizational skills, interpersonal skills, good judgment, staff management and staff productivity.  Ideal candidate would be skilled in Quality Assurance as well as Utilization Reviews. 

 

Job duties include putting together around 50 Treatment Authorization Requests (TAR’s) a month to Medi-Cal, Chart Auditing, Staff and Participant supervision, and communication with state officials.

 

Other job responsibilities include:

1)      The Program Director is directly responsible to the Administrator for the achievement of the position responsibilities. 

2)      Oversee the process from the first point of contact with the prospective participants; screening of applicants and the preliminary decision on eligibility.

3)      Assist the Administrator in management and administrative responsibilities in accordance with licensing requirements.

4)      Operate and manage the Center program, interpret recognized standards of care, supervision and management of the facility

5)      Supervise and coordinate employed and contract professionals working at the Center’s Multidisciplinary Team to implement the program

6)      Verify that services are delivered and documented according to the individual plan of care.

7)      Make staffing, training and development recommendations to the Administrator.
