Case Manager - Licensed Social Worker - PT

Mosaic Community Services, Inc., a Maryland leader in the community, Mental Health services industry, is seeking to fill a Case Manager opening at one of our residetnial, Adolescent - Therapeutic living programs based on Cockeysville, MD.

**Must be a Licensed Social Worker to be considered for this opening.(LGSW, LCPC, etc.)

 

20 hours per week for $20,000 Salary.

 

Duties:

For an assigned caseload of independent living clients and under general supervision, provides case management services with the goal of supporting the client’s successful community integration.  Completes an initial assessment, develops with the client an individual rehabilitation plan in conjunction with other providers of services and updates this plan.  Completes required record keeping and provides a safe environment according to the policy and procedures.

 

Requirements:

 

1.  Masters Degree in Social Work, psychology, OR related discipline is preferred, and/or (2+) years of direct job related work experience;

 

2.   Have at least 2 years experience working with adolescents;

 

3.  Be State licensed as a graduate or certified social worker;
