Case Manager, Developmental Disabilities

Position Summary:



The Residential Case Manager works as part of a team to provide direct supports, guidance, education, advocacy and supervision for individuals with developmental disabilities in accordance with the mission of Riverside and all state regulations.



The Residential Case Manager takes a leadership role for both identified individuals and the household, helps individuals live safely in the community, maintain their physical and emotional health, foster relationships with housemates, family and community and develop the skills necessary to increase the individual’s independence. Interaction between staff and individuals will reflect an appreciation of each individual’s uniqueness.



As a member of the team, the Residential Case Manager supports the human rights of individuals through the development of self-worth, empowering individuals to become the primary decision-makers in their lives. They offer a listening and counseling presence for individuals with questions and concerns (programs may have checklists that identify the specific tasks that each staff person is expected to complete).



Residential Case Managers are assigned specific responsibilities to identified individuals. These responsibilities include the following:

Conducting and writing the assessment; responsible for the development, implementation and revision of the Individual Service Plan for identified individuals; ensures that individuals receive appropriate medical care by setting up doctor and other health related appointments; maintains confidential file; writes progress notes; assists the individual in managing their funds; completes monthly documentation.


 

Full and part time positions available in Wakefield, Peabody, Woburn and Reading, MA.


 

Requirements:

Bachelor’s degree preferred with 1-2 years experience working with individuals with developmental disabilities.

Valid drivers license required.
