Case Manager-Community Rehab Services
Dimock Community Health Center seeking a Case Manager to work with clients with chronic and persistent mental illness. The Case Manager assists clients improve their daily living skills, manage their finances, increase medication compliance, develop job and social skills, develop leisure and time management skills, and secure stable, safe and affordable housing. The case manager will participate in interagency and interdisciplinary team meetings at multiple locations including hospitals, clinics and client’s residence.
Candidates must have excellent written and oral abilities, good observational skills and Massachusetts Drivers License. Own transportation essential.
Educational Requirements:
Bachelor’s degree in a social service discipline, e.g. psychology, social work, education.
