This position manages and coordinates resources necessary and appropriate for the achievement of optimal clinical and financial patient outcomes. Minimum 2 years experience in case management, discharge planning, and/or utilization review within the last 5 years. Bachelors degree with 3 years clinical experience in an acute or home health setting; or RN with 5 years clinical experience in an acute or home health setting. Quality management, community health or home health coordination experience desirable. Valid CA RN license.
