Care Manager Supervisor

JOB TITLE:  Care Manager Supervisor
REPORTS TO:  Director of Operations
SUPERVISES:  Care Managers                    

QUALIFICATIONS:Embraces the mission, philosophy, and values of the Partnership for Children of Essex and demonstrates an ability to work in collaboration with the various individuals and systems involved in the CMO effort Master’s Degree in a relevant discipline (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education) with a minimum of two year’s post Master’s related supervisory experience in child welfare, children’s behavioral health, juvenile justice, special education, or a related public sector human services or behavioral health position working with at risk children and families.  Professional license preferred. Bilingual a plus. Experience in clinical assessment and child/adolescent development, with community based experience preferred Clinically and culturally competent, with the training and experience necessary to manage complex cases across child serving systems Knowledge of the Essex County community preferred Computer literate with excellent writing skills Excellent public speaking skills Ability to work with energy and enthusiasm on a successful team Experience with community relations and resource development a plus Must possess a valid driver’s license Prior Wraparound Child and Family Team experience a plus Must pass a criminal background check

 

RESPONSIBILITIES:Advance the mission and values of the Partnership for Children of Essex on behalf of staff, children and families Supervise Care Managers in the discharge of their duties Coordinate the effective production of high quality service and resource management within the Partnership for Children of Essex Provide formal and informal presentations throughout the community on the missions and goals of the Partnership for Children of Essex Plan and coordinate service utilization, needs, gaps, and linkages in conjunction with the community Resource Manager and Assistant Serve as role model to Care Managers, other PCE employees, and the children and families Adequately staff Care Management units by participating in the hiring and performance assessments of Care Managers. Plan, organize, and facilitate in-service training for Care Managers and others Plan, design, prepare, and analyze reports consistent with established database and defined Quality Management System Provide critical information and analysis and creative ideas to Senior Management and the Board of Trustees of the Partnership for Children of Essex regarding quality service delivery and resources
Serve as liaison to the Operations Director on matters directly relating to child and family services, and those requiring approvals

PCE is a care management organization that uses a wraparound model of care to design, implement, and manage Individual Service Plans (ISPs) that are specific to a child's individual strengths and complex needs.  These ISPs are developed by using a Child and Family Team process.

The wraparound  model of care used by PCE is an approach to helping children that is family friendly and community based.  Emphasis is placed on the strengths of the child, family, and community.

A primary goal of PCE is to develop creative individualized service plans that help keep youth at home, in school, and out of trouble.
