Care Coordinator / 766 Position Description:Summary: Position will assess the behavioral health care needs of the Medicaid SMI/SED population for the Community Care Services (CCS) program. They will link CCS members with their BHP and PCP. They will also coordinate member referrals to appropriate community services. Participates in on-call 24 hour rotation for emergency response.Education:Bachelor’s Degree in a social science field preferredExperience:Minimum of two years related experience with mentally ill population.Knowledge of hospital procedures governing the discharge of mentally ill. Ability to maintain complete and accurate member records and to establish rapport and work effectively with SMI members. Independent, highly organized team player, able to handle multiple tasks simultaneously. Excellent communication skills and computer literate. Must have car and clean driving record.
