CLINICAL STAFF PHARMACIST

CLINICAL STAFF PHARMACIST Clinical Staff PharmacistRotating Shift (1st and 2nd), Weekday/Weekends (1 weekend/month)CLINICAL STAFF PHARMACISTWill work on the med surg units to provide clinical pharmacy services to patients and the nursing and medical staff. Responsibilities would include, but not limited to:Review New Admission orders and reconcile medicationsIV to PO conversion programRenal dosingESA dosing and monitoringTherapeutic Drug monitoringAntibiotic stewardshipRound with physiciansPatient Discharge counseling.Pharm. D with 3 years experience preferred, but will consider a registered pharmacist with appropriate clinical experience.Weekend coverage and some staffing will be required.Looking for a self confident team player with a solid clinical background to grow existing clinical program.Assures appropriate and timely drug delivery through evaluation and processing of medication orders, dispensing of medications, and supervision of support staff in accordance with standards of practice, legal requirements and policies and procedures of the pharmacy department. Serves as a resource for drug information and acts as a patient advocate regarding safe, effective and cost-effective drug therapy. Education, Training and Experience: B.S. in Pharmacy or Pharm D or a Foreign Equivalency ExamLicense, Registration and Certification: Must currently be a Registered Pharmacist in the State of PA or Licensure Eligible www.mercyhealth.org
