CLINICAL SERVICES CARE COORD - ST5258
Manage and coordinate the care of individuals with complex needs. Will work to assess, plan, implement, coordinate, monitor, evaluate options and services to meet a members needs. Communicate with members, providers, community agencies to develop appropriate treatment plans. Requirements: Bachelor level Clinician with five years of direct service experience in the delivery of substance abuse treatment services for adults. Demonstrated ability to work collaboratively with providers, community and state agencies, and other agency personnel. License and AAPS certification preferred.
