* SHIFT HOURS: 6:30am-2:30pm OR 2:30pm-10:30pm * POSITION SUMMARY: The Certified Home Health Aide / Certified Nursing Assistant is a paraprofessional member of the health care team who works under the supervision of a skilled nurse to perform personal care tasks as assigned. MAIN DUTIES AND RESPONSIBILITIES (Essential Functions of the Position): 1. CHHA may perform the following under the supervision of a nurse: personal hygiene including bathing, hair care, oral hygiene, skin care, positioning and range of motion as needed.2. Care given to patient according to instruction on the CHHA assignment sheet/plan of care.3. Assists with ambulation and transfer activities as needed. 4. Assists with meal preparation and assist with feeding as needed. 5. Provides for safe and sanitary environment for patient, such as: change linens, dusting, vacuuming, cleaning patient's bedroom, bath, kitchen, and laundry, as needed.6. Accompany patient to appointments and outings as assigned. Transports patient in patient vehicle only. CHHA must have valid California Driver's License on file in agency in order to transport.7. Completes CHHA clinical note and submits to agency weekly.8. Documentation of CHHA reflects care given to patient according to plan of care/assignment sheet. Any changes in condition are reported promptly to Nursing Supervisor.9. Meets safety needs of patients and uses equipment properly and safely (bed rails, restraints, oxygen, foot stools, etc.) per agency policy.10. Availability is given weekly and commitments are honored.11. Reports scheduling changes to Staffing Coordinator as needed, per agency policy.12. Provides care at scheduled times and days of the week.13. Available to work at least one weekend per month and three (3) holiday shifts per year if needed.14. Calls office if shift replacement has not arrived at scheduled time and stays with patient until replacement arrives. JOB LIMITATIONS: 1. May not administer medications2. May not give nasogastric tube feedings.3. May not irrigate Foley catheters.4. May not perform skills or procedures requiring the knowledge, training, and skill of a licensed nurse.5. May not perform any sterile procedures or intravenous procedures.6. May not write orders from a physician. Refers to agency as needed.7. May not transport patient in the employee's car.8. May not perform heavy housework or heavy cleaning.9. May not trim finger or toenails. EXPERIENCE/TRAINING/EDUCATION/CERTIFICATION:Required: ? High school diploma or equivalent general education degree (GED).?Completion of approved CHHA training program with six months of experience.? Current BLS-CPR Certification. SKILLS/ABILITIES:? Ability to communicate effectively with internal/external customers.?Ability to establish and maintain a courteous and service-oriented working relationship with patients, families, visitors, hospital staff, and co-workers.? Use of effective English, grammar, and spelling.? Exercise excellent time management and organizational skills to meet deadlines and sustain ability to work in a multi-task environment.? Ability to work in a rapidly changing and ambiguous environment.? Willingness to take on new projects or a history of taking on new projects.? Ability to fluently speak, read, write, and understand the English language.
