Professional CASE MANAGER Footprints Carolina is seeking individuals with strong Targeted/ CAP Case Management skills, to join our growing team. We are currently looking for Case Managers for our Gastonia, Lincolnton, Shelby, and, Conover offices. Individuals selected for this key role will be responsible for overall care coordination in accordance with person-centered practices and principals. Candidate must possess a Bachelor's degree in a Human Services field with 2+ years post degree experience. QP status required.
