Bi-Lingual Clinical Health Specialist

Job Description:POSITION SUMMARY Under the direction of the Manager, this position is responsible for completing member engagement tasks such as obtaining health histories, basic health risk assessments and member education using pre-defined tools, all according to program standards of care. These services are performed telephonically in a call center setting. The LPN (CHS) supports the objectives of the Disease Management program. Together with the Care Managers, Program Managers, and Clinical Support Specialist the LPN (CHS) ensures that members receive optimal service during their participation in the program. GENERAL RESPONSIBILITIES * Performing outbound calls and receiving inbound calls to conduct member interviews using an integrated telephone and computer system. * Educates members on the value of the program and the services they will receive as a program participant * Forges collaborative relationships with members and their family members / significant others over the telephone to promote optimal disease management and self-management. * Educates members and family members / significant others on treatment objectives and recommended interventions as per national guidelines using pre-defined tools utilized by the disease management programs. . * Works effectively as a member of the Disease Management Team (both clinical and non-clinical), to support members in the Disease Management program and coordinate needed services. * Forwards clinical issues or concerns to RN Care Managers or RN Program Managers for clinical oversight and consultation and implements recommendations as per their recommendations. * Other duties as assigned by Manager or Supervisor. REQUIRED EDUCATION AND EXPERIENCE * Valid LPN license with no restrictions in the state where assigned. * 3 years clinical experience preferred. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES * Familiar with the disease processes for the managed disease states, e.g. diabetes, congestive heart failure (CHF), end-stage renal disease (ESRD), and chronic obstructive pulmonary disease (COPD) * Excellent verbal and written communication skills. BI-LINGUAL (SPANISH/ENGLISH) REQUIRED * Works well in team environment * General office skills (faxing, copying, scanning, mailing, computer keyboard proficiency). * Excellent organizational skills. * Good knowledge of Microsoft Office Suite (Word, Excel, and Outlook) required. . * Excellent communications skills are a must. * Flexible and adaptable to changing tasks.
