Admission Nurse (Hospice) in Beachwood

At VITAS, the nation’s leading hospice provider, established in 1978 and currently operating 43 programs in 15 states, you’ll be empowered to always put the needs of patients and their families first.  We’ll support your efforts in every way possible and reward your commitment with competitive compensation, flexible schedules and comprehensive benefits, including tuition reimbursement, 401(k) with a match and a generous paid time off plan. As part of our interdisciplinary team, you’ll be the pivotal person who provides care, compassion and support to out patients and their families. The Admission Nurse is a RN primary responsibility is to support the development of appropriate referrals by creating and sustaining business relationships with referral sources in the assigned hospital accounts and to present VITAS services and philosophy to patients and families in a manner that results in the admission of appropriate patients.  Responsibilites are as follows: Â· Meet with patients and families to educate concerning VITAS’ services· Candidates must also have the capacity to learn the VITAS information system· Must be able to demonstrate the capacity to learn clinical and VITAS pain and symptom management approaches· Coordinates care plan for complicated clinical cases· Previous oncology experience preferred· Good interpersonal and customer service skills· Knowledge of pain management/palliative care QUALIFICATIONS:Experience in service/sales in health care environment.
