Qualifications. The ICU RNs shall possess and maintain the minimum qualifications stated below:
REGISTERED NURSE
Education as follows: Either (a), a Bachelor’s Degree in Nursing; or (b) a Diploma Graduate in Nursing; or (c) an Associate Degree in Nursing from an accredited university.
A current unrestricted license to practice as a registered nurse in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
Be a US Citizen by birth or naturalization
In addition, the following qualifications are required:
a. A minimum of three years of experience (40 hours per week) within the last five years caring for adult patients as an RN in an Intensive Care Unit with at least one year in a tertiary care facility. Experience to include intra-aortic balloon pump proficiency.
b. Successful completion of a critical care training program acceptable to the American Association of Critical Care Nurses (AACN) or equivalent experience.
c. Maintain current certification in Advanced Cardiac Life Support (ACLS).
Full-time AND Per-diem ICU RNs for 12 hour day shifts and 12 hour night shifts seven (7) days a week

REGISTERED NURSE DUTIES
Perform a full range of RN duties including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians’ orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.
Provide professional nursing care in a knowledgeable, skillful, consistent, and continuous manner.
Assess each patient and perform triage duties as assigned.
Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
Evaluate effectiveness of self care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.
Provide treatment and discharge instructions upon patient release.
Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
Provide a safe and clean environment for each patient.
Ensure areas are stocked and properly equipped.
Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
Assist in planning, supervising and instructing LPNs/LVNs, nursing assistants and ancillary personnel
and the following duties:
a. Perform in accordance with established principles, practices, and ethics of the nursing profession and written policies, procedures, and requirements of NNMC Bethesda. The standards of performance for all health care workers shall reflect the degree of care, skill, and learning expected of a reasonably prudent health care worker in the professional category to which he/she belongs.
b. Render professional nursing care to patients within assigned unit of the hospital.
c. Establish priorities of patient care based on essential patient needs and available unit resources including time, personnel, equipment, and supplies.
d. Demonstrate competency in medication and intravenous (IV) administration to include blood products and Total Parental Nutrition (TPN).
e. Perform documentation duties on unit and in patient record which are timely, accurate, and concise.
f. Demonstrate sound knowledge base and actions in the care and decision making for designated patient populations.
g. Participate actively in staff development for unit and Directorate personnel.
h. Demonstrate self directed learning and participation in continuing education to meet own professional growth.
i. Seek validation of knowledge base, skill level, and decision making as necessary and assertively seeks guidance in areas of question.
j. Participate in development and attainment of unit and Nursing Directorate goals.
k. Demonstrate effective and professional communication methods, and skills, using lines of authority appropriately.
l. Formulate and use effective working relationships with all health care team members, patients, and significant others.
m. Practice effective problem identification and resolution skills as a method of sound decision making.
n. Once oriented, act as the charge nurse or relief charge nurse or team leader when directed by the Director of Nursing Services or his/her designated representative. Contractor nurses will only be required to perform the functions of the charge nurse in the absence of both the military and civil service charge nurses.
o. Review and make appropriate notation on the chart of a patient at least once in an 8 hour shift, or per unit specific requirements.
p. Perform other duties within the scope of RN practice as requested.
q. Once provided with "hands-on" experience, demonstrate working knowledge and use of computerized systems, especially CHCS/Armed Forces Health Longitudinal Technology Application (AHLTA), AWMS and CIS.
r. Operate cardiac monitors, interpret cardiac rhythms, determine implications, and make indicated interventions.
s. Administer prescribed medications to include IV push drugs and IV drips. Monitor and document patient response.
t. Care for patients on ventilators as well as patients requiring pacemaker insertion and subsequent monitoring and those on balloon pumps.
u Operate hemodynamic equipment safely and effectively. Interpret generated data and respond appropriately.
v. Care for dialysis patients.
w. Successful completion of a coronary care training program and a critical care course acceptable to NNMC (course/certification) including a basic 12 lead EKG interpretation course.
x. Cross trained to all Adult Critical Care areas, including the Cardiovascular Inpatient Telemetry Ward (3W).
y. Provide highly specialized nursing care to critically ill patients who require close observation for subtle changes in condition. Provide highly specialized nursing care to widely varied patient populations (e.g., cardiovascular surgery, neurosurgery, etc.).
z. Recover stable and unstable patients from general and regional anesthesia. Administer and monitor oxygen therapy, and interpret and respond to data. Utilize intensive pulmonary care techniques as needed.
aa. Respond decisively to life-threatening changes in patient condition, providing immediate and appropriate emergency measures according to ACLS protocols. Assist in the direction and coordination of unit CPR and ACLS efforts. Respond to cardiac arrest situation outside special care units as designated critical care representative to facilitate resuscitative efforts. Promote staff competency and efficiency in emergent situations through formal/informal classes.
bb. Operate and monitor the performance of sophisticated equipment used in the care of critically ill patients, including but not limited to: mechanical ventilators, hemodynamic monitoring equipment, intracranial pressure monitoring, intra-aortic balloon pump, etc. Interpret and respond to complex technological data generated by such equipment.
cc. Administer a wide variety of rapid-acting highly potent medications (direct intravenous and other routes) according to unit policies, procedures and protocols. Utilize a broad knowledge of drugs and intravenous therapy, including properties, indication, dosages/rates and side-effects. Closely monitor and document patient response, and adjust therapy accordingly.
dd. Plan, direct and coordinate total patient care activities and multidisciplinary involvement by other intra- and inter-service lines. Initiate/coordinate regular periodic Patient Care Conferences. Assess and plan for patient’s readiness for transfer or discharge. Coordinate transfer of long-term critical care patient to general ward or home. Function as primary liaison/advocate for patient and family, utilizing appropriate crisis intervention techniques.
ee. Participate in multifaceted staff development activities at the unit and Command level including but not limited to the following activities:
- Teach formal and informal unit/area classes.
- Instruct in Command BCLS/ACLS and specialty nursing care courses.
- Participate in the orientation and training of new unit personnel, including interunit cross training.
- Identify individual/group staff development requirements, and establish formal/informal training programs to meet specific needs.
- Function as a role model/mentor for experienced professional staff by teaching highly specialized advanced nursing concepts and difficult and complex nursing procedures.
- Maintain professional competency through active participation in: Independent study, nursing preceptorship, in-service classes, multidisciplinary or nursing grand rounds, seminars/symposiums,
cross training/rotational assignments.
ff. Function as relief charge nurse, primary nurse, or team leader responsible for direct patient care and technically direct assigned civilian and military professional and paraprofessional staff.
gg. Assign nursing staff according to the complexity or difficulty of patient care requirements and the qualifications and capabilities of staff member.
hh. Hold periodic conferences and counseling sessions for professional and paraprofessional staff regarding patient care. Participate in the performance evaluation process.
ii. Technically direct the work of health care trainees, temporary nurses and medical personnel to ensure adherence to unit standards of practice.
jj. Maintain required records and documentation, including statistical records of workload.
kk. Function as primary patient contact representative to resolve complaints/conflicts regarding nursing-related issues, working relationships or unit conditions. Refer unresolved complaints and infraction to charge nurse with recommendations for appropriate action. Promote harmonious, positive interpersonal relationships with patients, families and other health care personnel. Direct particular attention to supporting emotional needs in a demanding and stressful critical care environment.
ll. Utilize knowledge of critical care ethical and legal issues to provide maximal patient/family support during periods of physiopsychosocial stress. Provide appropriate crisis intervention.
mm. Adhere to and promote unit Performance Improvement programs, including unit specific monitoring and evaluation of appropriateness of care.
nn. Promote patient and staff safety through education and adherence to accepted safety and infection control standards. Maintain a clean, safe, orderly and therapeutic working environment.
oo. Participate in multidisciplinary special projects or studies which impact on nursing. Utilize methods to share findings of research with nursing colleagues and others.
pp. Perform additional duties within scope of RN practice including but not limited to: Performance Improvement, Standards of Nursing Care, Command Instructions, Special Care Unit Manuals, and other collateral duties required for efficient quality patient care.
qq. Successful NNMC competency in the following areas:
(1) Mechanical ventilation; ABG interpretation; and extubation.
(2) Hemodynamic monitoring.
(3) Critical care medications (IV push and continuous infusion of vasoactive medications).
(4) Intracranial Pressure monitoring.
(5) Moderate Sedation
(6) Nursing/Command orientation, initial and annual training requirements
