Physicial Therapist - PT

Physical Therapist services to be performed which include, but are not limited to:

 

Evaluates assigned, referred outpatients, domiciliary patients, acute inpatients, or long-term care patients.

 

Evaluates for the presence of physical abnormalities, determination of function-related problems, including the basis for making the assessment, development of individualized PT care plans, use of assessment data to plan individualized treatment programs, choice of therapy technique best suited to condition of the patient to achieve functional results.

 

Treats with therapeutic exercises, joint mobilization, lower extremity prosthetic and orthotic assessment, fitting and training, neuromuscular re-education, gait analysis and training, functional activity training, pain management patients, physical agents, participates in wheelchair assessment and fitting, and issuing of ambulation devices for unscheduled patients. Treatment emphasis is on maximizing function, both in the psychosocial and physical restorative areas.

 

Assesses effectiveness of treatment daily, and modifies as necessary, always using appropriate graded treatment modalities/activities.

 

Completes electronic consults and/or progress notes on patients assigned, at appropriate intervals, including observations of patients' functional status, behaviors pertinent to rehabilitation, relevant short and long-term goals, progress towards discharge, and disposition recommendations.

 

Initiates or recommends referrals for follow up treatment and/or specialized assessment by physicians or other health care providers.

 

Provides patient/family education and home treatment programs with instructions for independent therapy, to include safe therapeutic practices.

 

Provides patient care which is appropriate to the cognitive, emotional, and maturation needs of the adult and/or geriatric patient to include demonstration of the:

a. knowledge and skills necessary to provide care appropriate to age of the patient served;

b. knowledge of changes associated with aging; and

c. abilities to provide care based upon age related factors.

 

Qualifications:

 

An active, unrestricted Physical Therapist License to practice in any state

Graduate from an accredited physical therapist education program

Minimum twelve months experience

Basic Life Support Certification

 

Work Hours: Monday through Friday, 8:00am – 4:30pm
