Job Description:Conduct one-on-one and group outreach presentations to recipients Assist other Outreach Counselors with daily management duties Present program education and enrollment options to recipients. Present managed care options to recipients and assist them in using their health care plans Maintain positive relationships with county staff, state agencies, community groups and recipients Complete direct data entry into designated internal systems in accordance with IEE Project policies and procedures Conduct telephone calls and home visits in accordance with project policy and procedures. (75% of the time) Knowledge of community and participates in community events, health fairs and other activities as needed. Responsible for reporting outcomes on a daily/ weekly basis Willingness to work a flexible schedule including weekends and evenings Perform other duties as may be assigned by management
Education/Experience:High School diploma, GED, or equivalent certification Bachelor’s degree in social work preferred Six months of human services experience, preferred Knowledge of the community to be served Proficiency with MS Office Suite is required Proven excellent organizational, interpersonal, written and verbal communication skills Ability to follow directions Ability to perform comfortably in a fast-paced, deadline-oriented work environment Ability to successfully execute many complex tasks simultaneously Ability to work as a team member, as well as independently Dealing with the public Fluency in commonly spoken languages Travel in assigned area up to 70% if time. Candidate must have reliable transportation, auto insurance in accordance with The State of Texas requirements, and a valid Texas state driver’s license Travel to client dwellings which may include rural homes and multilevel apartments Ability to negotiate multiple flights of stairs on a routine basis. This position requires bending, stooping, pushing, pulling, lifting and carrying weights up to and including 25 pounds
